1. What is lymphoma?<\/strong><\/p>\n\n\n\n

Cancer of lymphocytes, lymphocytes are very important infection fighting cells in the body<\/p>\n\n\n\n

2. How these patients present to a doctor<\/strong><\/p>\n\n\n\n

Swollen lymph nodes sometimes painful, commonly neck, axilla, groin<\/p>\n\n\n\n

Fever<\/p>\n\n\n\n

Weight loss<\/p>\n\n\n\n

Loss of appetite<\/p>\n\n\n\n

Night sweats that soak your clothes<\/p>\n\n\n\n

3. How many type of lymphoma are there<\/strong><\/p>\n\n\n\n

Broadly they are divided into Hodgkin and Non-Hodgkin lymphoma<\/p>\n\n\n\n

Hodgkin\u2019s lymphoma<\/p>\n\n\n\n

\u2022 Classical<\/p>\n\n\n\n

Mixed cellularity<\/p>\n\n\n\n

Lymphocyte predominant<\/p>\n\n\n\n

Lymphocyte depleted<\/p>\n\n\n\n

Nodular sclerosis<\/p>\n\n\n\n

\u2022 Nodular lymphocyte predominant<\/p>\n\n\n\n

In NHL many subtype<\/p>\n\n\n\n

\u2022 DLBCL<\/p>\n\n\n\n

\u2022 Follicular lymphoma<\/p>\n\n\n\n

\u2022 Mantle cell lymphoma<\/p>\n\n\n\n

\u2022 Anaplastic large cell lymphoma<\/p>\n\n\n\n

\u2022 Burkitts lymphoma<\/p>\n\n\n\n

\u2022 Lymphoblastic lymphoma Etc<\/p>\n\n\n\n

4. How lymphoma is diagnosed<\/strong><\/p>\n\n\n\n

Most important part of lymphoma diagnosis is biopsy and immunohistochemistry<\/p>\n\n\n\n

Once the diagnosis is confirmed, staging workup is done with PETCT scan, BMA and BM biopsy,<\/p>\n\n\n\n

5. What are the other test done for lymphoma?<\/strong><\/p>\n\n\n\n

Routine tests like Complete blood picture, liver function test and renal function test. Virology, 2 D echo,<\/p>\n\n\n\n

6. How they are treated?<\/strong><\/p>\n\n\n\n

Lymphomas are treated based on the type of lymphoma and stage<\/p>\n\n\n\n

Early stage lymphomas \u2013 e.g. follicular lymphoma stage 1 and 2 without symptoms are observed<\/p>\n\n\n\n

Stage 1 and 2 Mantle cell lymphoma are observed<\/p>\n\n\n\n

Hodgkin\u2019s lymphoma are with ABVD Protocol<\/p>\n\n\n\n

All high grade lymphomas are treated with chemotherapy \u2013 RCHOP protocol is used<\/p>\n\n\n\n

Symptomatic Low grade lymphomas are treated with BR protocol<\/p>\n\n\n\n

7. Is there any role for radiotherapy?<\/p>\n\n\n\n

Yes,<\/p>\n\n\n\n

a. in early stage tumours radiation can be used, based on site and histology<\/p>\n\n\n\n

b. At the end of chemotherapy, if PETCT scan shows disease activity in lymph nodes then radiation can be considered for the PETCT positive lymphomas<\/p>\n\n\n\n

8. Can lymphomas be observed without treatment\/chemotherapy?<\/strong><\/p>\n\n\n\n

Yes \u2013 low grade lymphomas like follicular lymphomas grade I and II and early stage mantle cell lymphoma can be observed<\/p>\n\n\n\n

9. Is cure possible for lymphomas?<\/strong><\/p>\n\n\n\n

Stag 1 and 2 Hodgkin\u2019s lymphoma and NHL<\/p>\n\n\n\n

Few stage 3 and stage 4 also possible<\/p>\n\n\n\n

10. If they relapse how they are treated<\/strong><\/p>\n\n\n\n

Hodgkin\u2019s and non-Hodgkin\u2019s lymphomas are treated with salvage chemotherapy protocols which are same for both the type of lymphomas<\/p>\n\n\n\n

a. DHAP,<\/p>\n\n\n\n

b. GDP<\/p>\n\n\n\n

c. ICE  etc<\/p>\n\n\n\n

11. Is there any role of bone marrow transplant in lymphoma?<\/strong><\/p>\n\n\n\n

Yes \u2013 when lymphomas relapse, they are treated with chemotherapy and then they are consolidated with bone marrow transplant.<\/p>\n\n\n\n

12. What are the new drugs available for lymphoma?<\/strong><\/p>\n\n\n\n

For Hodgkin\u2019s lymphoma<\/p>\n\n\n\n

a. Brentuximab<\/p>\n\n\n\n

b. Immunotherapy \u2013 nivolumab and pembrolizumab<\/p>\n\n\n\n

c. CAR \u2013 T CELL therapy<\/p>\n\n\n\n

For NHL<\/p>\n\n\n\n

CART cell therapy<\/p>\n\n\n\n

d. Axicabtagene ciloleucel<\/p>\n\n\n\n

e. Tisagenlecleucel<\/p>\n\n\n\n
